AEHC Volunteer Agreement Form
This completed form is to be filled out and turned into the county office so that agents or admins can enter the data into the EHC Membership Database at ehc.uada.edu. Alternatively, it can be sent to the EHC Administrative Specialist at the state office to enter. Each member only needs to fill this form out once, when they become a member.
Annual Forms due to State Office with August 1 Deadline
Annual Financial Report
Deadline for this report to the EHC Administrative Specialist at the state extension
office is August 1. This form can be utilized for club/council. You may also submit
a copy of your financials in the format you have chosen to use as long as it gives
the same information.

AEHC Peer Review of Group Financial Records
Each club/council is to perform a peer review annually as part of the financial records.
A copy of the completed financial review is to be mailed to the state office to the
attention of EHC Administrative Specialist no later than August 1.
Project Books
Project Books are due Oct 31 each year. You may submit them at District Rallies or mail them to the Little Rock State Office to the attention the EHC Administrative Specialist.

Scholarship and Mini-Grants
AEHC Education Mini-Grant Application Guidelines
Applications should be addressed to the AEHC Administrative Specialist and recieved by March 1.
Send to the AEHC Administrative Specialist by April 15 each year. Applications must be sent complete with candidates essay, a copy of the candidate's latest transcript, and any other additional supporting documents the candidate feels is needed to support application questions.

Each county may submit one nominee and members of the Past Presidents Organization
will make the
fnal decision. Mail application by May 15th to: EHC Administrative Specialist at the
State Office, 2301 S. University Ave., Litle Rock, AR 72204.
