C.E.S.P. 3-40: Release and Waiver of Liability

Summary: Establishes policy requiring written release and waiver of liability for participants in 4 - H Center ropes course, adventure and challenge camp and other hazardous activities.

A signed release and waiver of liability must be obtained for each participant in the 4 - H Center ropes course, 4 - H adventure and challenge camp, and other "high adventure" activities involving hazardous activities. Parental waivers must be obtained for participants under the age of 18.

The signed release and waiver or liability must be in the hands of an authorized employee of the Cooperative Extension Service before participation in the activity begins. In the case of minors who fail to bring the release form to the activity, a parent or guardian may complete a form at the county Extension office and notify the person in charge of the activity that the form has been filed.
